Radical pleurectomy and chemoradiation for malignant pleural mesothelioma: The outcome of incomplete resections

The type of surgery (radical pleurectomy (RP) vs. extrapleural pneumonectomy (EPP)) remains controversial for malignant pleural mesothelioma (MPM). Macroscopic complete resection (MCR) is a key prognostic factor. It is unclear, if patients undergoing incomplete RP within a standardized multimodality treatment protocols have any advantage in terms of survival and if EPP could theoretically have avoided incomplete resections (R2).Eighty-eight patients underwent RP followed by chemoradiation from 2002 to 2011 within a prospective multimodality treatment study at a single institution. Present research concerning the efficiacy of therapeutic strategies for squamous cell carcinomas of the oral cavity and the oropharynx profits from former observational studies. The so-called retrospective German, Austrian, and Swiss Association for Head and Neck Tumors (DOSAK) study (Platz et al. 1986) served to review existing proposals for classification (UICC 1973, 1978; AJC 1978) and to develop a statistical classification which enables the head and neck oncologist to define prognostically homogeneous patient collectives. The treatment-dependent prognostic index (TPI) was the corresponding result (Platz et al. 1982).

Resume Introduction Le cancer colorectal represente la 3e cause de mortalite par cancer : 5 % des patients vont developper des metastases pulmonaires. Le concept de chirurgie des cancers oligometastatiques est base sur l’objectif d’un controle local optimal de la maladie. Etat des connaissances A ce jour, aucun essai controle randomise ne valide la chirurgie de resection des metastases pulmonaires des patients oligometastatiques porteurs d’un cancer colorectal controle localement.
